Visitation Guidelines 3/30/21



We are excited to announce that we are able to offer visitation for our Residents once again! Visits can be scheduled by calling 724-458-7760, please speak with Linda Garner or Laura Cook.  We appreciate all of your patience and understanding as we move forward together.    Listed below are our visitation guidelines to follow. Enjoy your visit!

  1. Visitation hours will generally be Monday-Friday between the hours of 9:00am – 11:30am, and 1:00pm – 3:30pm. Families may be made aware of any additional timeslots via one-call.   Please understand all hours are subject to change.
  2. Visits must be scheduled at least 24 hours in advance and scheduled as available. All visitors must wear a mask during the entire visit.  Visits are limited for 30 minutes to allow visitation opportunities to all Residents.
  3. Only two visitors are permitted at a time. Children are allowed to visit when accompanied by an adult. The child will count as one of the 2 visitors and must be able to wear a mask for the duration of the visit if over the age of 2 years old. Adult visitors must be able to manage the child and the child must adhere to the strict social distancing of at least 6 feet.
  4. A designated staff person will assist in the visiting process by escorting the Resident to the designated visiting area. A designated staff person may observe/monitor the visit to ensure all precaution are being followed, while allowing an adequate degree of privacy. Once the visit is over, a designated staff person will assist the Resident back to his/her room as needed.
  5. The visitor/s must remain six feet apart during the visit, will sit in the assigned area and have a surgical/cloth mask on during the visit. The Resident will have a mask on for the entire visit unless unable to tolerate. If the RESIDENT is fully vaccinated (see definition below), the RESIDENT can choose to have close contact (including touch) with their visitor while wearing a well-fitting face mask and performing hand-hygiene before and after. Please note that the Resident must wear a mask the entire time of a close contact (including touch) visit.
  6. Visitors will physically distance themselves from other Residents and staff in the facility. Visitors will leave immediately after visitation is over.
  7. If at any time the CORE PRINCIPLES (See below) of COVID-19 infection prevention are not followed the visit will immediately cease and visitations may not be permitted to occur again.
  8. The Resident is to perform hand hygiene prior to leaving their room and traveling to the designated visiting area. The Resident is to perform hand hygiene at the end of their visit prior to or when returning back to their room. A designated staff person will assist the Resident with hand hygiene as needed.
  9. The designated staff person assisting with visitation will clean/disinfect the table, chairs, divider and any high touch areas.




  • FULLY VACCINATED:  Fully vaccinated refers to a person who is ≥ 2 weeks following receipt of the second dose in a 2-dose series, or ≥ weeks following receipt of one dose of a single dose vaccine, per the CDC’s Public Health Recommendations for Vaccinated Persons.
  • UNVACCINATED:  Individuals who do not meet ALL standards of the FULLY vaccinated definition above.


  • SCREENING of all who enter Orchard Manor for signs/symptoms of COVID (temperature and questions/observation for signs of COVID).
  • DENIAL OF ENTRY for those with signs/symptoms or those who have had close contact with someone with COVID infection in the prior 14 days (regardless of the visitor’s vaccination status).
  • HAND HYGIENE: (use of alcohol-based hand rub is PREFERRED).
  • FACE COVERINGS: Required at all times regardless of vaccination status. (cover mouth AND nose)
  • SOCIAL DISTANCING: at least 6 feet between persons.
  • VISITOR EDUCATION:  signage displayed related to expectations of visitors.
  • CLEANING/DISINFECTING:  cleaning process of designated visitation areas after each visit.
  • PPE USE:  proper use of PPE by staff and visitors.
  • COHORTING:  facility must have a plan in place to respond to COVID cases / COVID care.
  • TESTING:  Resident and Staff testing must be conducted as per testing requirements based on County positivity and facility positivity rates.

Visitors who are unable to adhere to the core principles of COVID-19 infection prevention will not be permitted to visit and will be asked to leave.


While taking a person-centered approach and adhering to the core principles of COVID-19 infection prevention, outdoor visitation is preferred even when the Resident and visitor are fully vaccinated against COVID-19. Outdoor visits generally pose a lower risk of transmission due to increased space and airflow. Therefore, visits WILL be held outdoors whenever practicable. However, weather considerations (e.g., inclement weather, excessively hot or cold temperatures, poor air quality) or an individual Resident’s health status (e.g., medical condition(s), COVID-19 status) may hinder outdoor visitsFor outdoor visits, Orchard Manor will utilize the front patio, as a safe outdoor space for visitation.  When conducting outdoor visitation, all CORE PRINCIPLES of infection control are to be followed.




The following scenarios will require INDOOR/OUTDOOR visitation to be RESTRICTED for specific Resident:

  • For Unvaccinated Residents:  if the Mercer County COVID positivity rate is greater than 10% AND less than 70% of Orchard Manor Residents are Fully Vaccinated.
  • For COVID positive Residents:  whether vaccinated or unvaccinated, will not be permitted visitors until they have met the criteria to discontinue Transmission-Based precautions.
  • For Residents in QUARANTINE:  whether vaccinated or unvaccinated, will not be permitted visitors until they have met criteria for release from Quarantine.


An outbreak exists when a new nursing home onset of COVID-19 occurs (i.e., a new COVID-19 case among Residents or staff). This guidance is intended to describe how visitation can still occur when there is an outbreak, but there is evidence that the transmission of COVID-19 is contained to a single area (e.g., unit) of the facility. To swiftly detect cases, Orchard Manor will continue to adhere to CMS regulations and guidance for COVID-19 testing, including routine staff testing, testing of individuals with symptoms, and outbreak testing.

When a new case of COVID-19 among Residents or staff is identified, Orchard Manor will immediately begin outbreak testing and suspend all visitation (except that required under federal disability rights law, end-of-life & compassionate care), until at least one round of facility-wide testing is completed.

Visitation can resume based on the following criteria:

  • If the first round of outbreak testing reveals no additional COVID-19 cases in other areas (units) of Orchard Manor, then visitation can resume for Residents in areas/units with no COVID-19 cases. However, Orchard Manor will suspend visitation on the affected unit until the facility meets the criteria to discontinue outbreak testing.  For example, if the first round of outbreak testing reveals two more COVID-19 cases in the same unit as the original case, but not in other units, visitation can resume for Residents in areas/units with no COVID-19 cases.
  • If the first round of outbreak testing reveals one or more additional COVID-19 cases in other areas/units of the facility (e.g., new cases in two or more units), then Orchard Manor will suspend visitation for all Residents (vaccinated and unvaccinated), until the facility meets the criteria to discontinue outbreak testing.

While the above scenarios describe how visitation can continue after one round of outbreak testing, Orchard Manor will continue all necessary rounds of outbreak testing. In other words, this guidance provides information on how visitation can occur during an outbreak, but does not change any expectations for testing and adherence to infection prevention and control practices. If subsequent rounds of outbreak testing identify one or more additional COVID-19 cases in other areas/units of the facility, then facilities should suspend visitation for all Residents (vaccinated and unvaccinated), until the facility meets the criteria to discontinue outbreak testing.

NOTE: In all cases, visitors will be notified about the potential for COVID-19 exposure in the facility (appropriate signage regarding current outbreaks within front lobby area), and adhere to the core principles of COVID-19 infection prevention, including effective hand hygiene and use of face-coverings.

Family Holiday Dinner Cancelled

Due to COVID restrictions, we regret to announce that the Annual Family Holiday Meal has been
cancelled for this year. As much as it saddens us, the safety of our Residents is our top priority.

Orchard Manor is fully committed to ensure our continual professional competence with compliance and code of conduct/ethics of not only Orchard Manor employees but that of independent contractors, volunteers and any vendors or other representatives of our facility. Dropping Off Items for Residents

Family members are permitted to drop off items (including flowers) for Residents during specified hours:
M-F 9:00am to 3:00pm; Sat & Sun 1:00pm to 3:00pm.

Everything must go through a 48-hour quarantine. Please keep this 48-hour quarantine in mind  and do not leave perishable food or anything that needs to be refrigerated. Remember to include the Resident’s name and date it was dropped off. If you have any questions about what can be left, please call 724-458-7760. Thank you.
